At Baptist Eye Surgeons, our top priority is the restoration and preservation of our patients’ vision and overall eye health.

We have compiled some general information you may find helpful as you prepare for your visit with us. If you are a new patient or haven’t been to Baptist Eye Surgeons in the last year, you may also wish to print these forms and fill out ahead of time. Please bring the completed forms with you on the day of your appointment.

Insurance and Billing Information

We want to make your visit as pleasant as possible. Remember that restoration and preservation of your vision is our first concern and we will be glad to work with you on insurance and payment matters. Please call our insurance specialist at 865-579-3974 to discuss your bill, insurance coverage, or financial hardship.
Please bring your insurance cards to each visit. Medicare, Medicaid, most TennCare, and most commercial insurance companies cover the majority of our medical procedures. Many insurance and Tenncare plans require a referral from your primary care physician. Please check with your insurance carrier to help obtain this referral prior to your office visit. If a valid referral is not received prior to your appointment time, you may be asked to reschedule.
Please be prepared to pay your out of pocket expense at the time of your visit. These expenses may include non-covered charges such as the refraction, co-payments, and deductibles. We accept most major credit cards: Visa, American Express, MasterCard, and Discover.

Accessing Your Accounts

You may access your patient records via our online patient portal or pay your bill via our online payment system. If you have any questions about accessing your account or paying your bill, please call our office at 865-579-3920.
